Dog boarding form

What the kennel asks for at check-in, filled in before you get there.

Check-in is the worst time to be looking for a rabies date. The facility has a queue, your dog is already unsettled, and the answer is in a photo somewhere in three years of camera roll.

Fill this in the night before and hand it over. Most kennels will happily take it in advance and pre-clear you — which turns drop-off into a signature instead of an interview.

Send their usual food. A diet change on top of boarding is a common cause of an upset stomach.

When the paper stops keeping up

PetPacket builds this as a Boarding Packet from records you already keep — vaccines with live expiry dates, feeding, medication and contacts — as a PDF the facility can read without installing anything.

Worth reading alongside it: dog boarding checklist, cat boarding checklist, and how to prepare for pet boarding.

Questions

About this template.

What to put in it, and what people usually leave out.

What do I need to bring to dog boarding?

Dated vaccination certificates, their usual food pre-portioned, any medication in its original packaging with a written schedule, and contact details for you, a backup human, and your vet. Most facilities also want written authorisation to treat and a cost ceiling. Leave anything irreplaceable at home.

Can I send this form to the kennel in advance?

Most will take it, and many prefer it — a facility that has your paperwork the day before can pre-clear the booking so check-in is a signature rather than a conversation. Email it or hand over a printed copy.

What is authorisation to treat?

Written permission for the facility to get your pet veterinary care without reaching you first, usually with a cost limit you set. Without it, staff may have to wait for you to answer the phone before a vet can act, which is exactly the delay you do not want. Name a second person who can authorise too.

Stop rewriting it

Fill it in once. Hand it over every time.

PetPacket keeps your pet's records in one place and builds the same page whenever you need it — current doses, current vaccines, current vet.
